You make a Medical Details declaration when booking - check the ‘Participant Info’ section of your confirmation/reminder email to see what your submitted. If you need to update this, please complete the form below.
Every participant must sign an Acknowledgement of Risk Form in advance of undertaking any element of their booking, in order to participate. You can find a link to complete this in your booking confirmation/reminder emails, via SmartWaiver. As outlined in the Acknowledgement of Risk Form, and as per our Terms & Conditions of booking, it is your responsibility to make us aware of medical conditions/injuries, past or present, (and/or medical/food allergies) which may affect your participation and/or compromise safety, or increase the risks associated with your booking.
Submitted forms are date/time stamped to prove receipt time and will be processed within Large Outdoors office hours.
